Join our dynamic healthcare team as a Nurse Practitioner specializing in Pulmonology within the Skilled Nursing Facilities (SNF) and Long-Term Care (LTC) settings. This 1099 independent contractor role offers an exciting opportunity to deliver comprehensive respiratory and general medical care to diverse patient populations. You will play a vital role in managing complex pulmonary conditions, coordinating care plans, and ensuring optimal patient outcomes through proactive assessment and treatment. Your expertise will directly impact the quality of life for residents in post-acute, sub-acute, and long-term care environments.
Responsibilities
- Conduct thorough clinical assessments, including vital signs, physical examinations, and detailed patient histories to evaluate respiratory and overall health status.
- Manage acute and chronic pulmonary conditions utilizing advanced skills such as spirometry interpretation, oxygen therapy adjustments, and ventilator management.
- Perform specialized procedures including thoracentesis, pulmonary function testing, and bedside sonography when necessary.
- Collaborate with interdisciplinary teams to develop individualized care plans that incorporate medication management, lifestyle modifications, and appropriate referrals.
- Administer treatments such as inhaler therapy, nebulization, IV infusions, and botulinum toxin injections for spasticity or other indications.
- Document all patient encounters accurately using electronic health record (EHR) systems like Epic or eClinicalWorks, ensuring compliance with HIPAA regulations.
- Educate patients and their families about disease management, medication adherence, smoking cessation, and lifestyle changes to promote health literacy.
Skills
- Proven experience working with patients in SNF, LTC, or post-acute care settings with a focus on pulmonary health.
- Proficiency in Epic EMR/EHR systems or similar platforms such as Athenahealth or Cerner for documentation and case management.
- Strong knowledge of respiratory therapies including ventilator management, oxygen titration, and airway clearance techniques.
- Familiarity with advanced procedures like thoracentesis, spinal tap (lumbar puncture), and dermal fillers is a plus.
- Experience with telehealth platforms to provide remote consultations and follow-up care effectively.
- Ability to perform comprehensive patient assessments involving physiology knowledge and diagnostic evaluation skills.
- Skilled in medication administration including injections, IV infusions, CPT coding for billing purposes, and understanding of ICD-10 coding standards.
- Knowledge of Workers' Compensation law related to occupational health cases is advantageous.
- Capable of managing complex cases involving infectious disease care, hospice & palliative medicine principles, or senior care needs such as dementia or Alzheimer's care.
- Excellent communication skills for triage calls, discharge planning, case management coordination, and family education.
