Job Description:

Practitioners provide advanced primary and specialty nursing services. Assessment, diagnosis, & treatment to all clients.

What does your role mean?

It means that you have the honor and the privilege to be invited into the personal homes of chronically ill & disabled patients. You get the opportunity to meet their caregivers, families, and learn their personal story, and ultimately you can provide interventions to those in need of your services & expertise and have the ability to change their quality of life for the better!

Job Responsibilities:

-Provide palliative and primary care to patients in their homes and assisted living.

-Perform physical exams and patient observations

-Record patient medical histories and symptoms

-Create patient care plans and contribute to existing plans

-Order, administer and analyze diagnostic tests

-Monitor and operate medical equipment

-Diagnose health issues and offer treatment options

-Detect changes in patients health and change treatment plan as necessary

-Administer medicines and treatments

-Evaluate response to treatments and medications

-Consult with healthcare professionals

-Training patients and families on managing illnesses or injuries and disease prevention

-Assist in the establishment of company goals and objectives

-Work with a variety of patients varying in age and health status

-Be friendly, courteous and sympathetic when it comes to working with patients and families

-Practice infection control standards at all times when working with patients and equipment per standards

-Follow all state, federal and/or local health and safety rules, laws and regulations

-Complete all mandatory training

-Maintain all necessary licensing/certification

-Provides excellent customer service, including handling calls/concerns in a professional manner
