The Role: Mid-Level Practitioner

We are seeking a part-time (average 20 hours per week) Mid-Level Practitioner to join our team and help us achieve our mission of changing lives through functional healthcare. The ideal candidate will have experience in functional medicine and hormone optimization and a passion for helping people achieve optimal health. Must have all state and national licensure up to date.

Role Outcomes:

• Successful management of patient care through program work and maintenance care, ensuring all patients receive the support they need to achieve their health goals.

Roles and Responsibilities:

• Conduct new patient visits and follow-ups, ensuring all treatment plans are being followed and making any necessary adjustments.
• Patient communication via the patient portal as needed.
• Order supplements and labs as required, ensuring all orders are accurate and timely.
• Track patient outcomes and gather testimonials and referrals to support the growth of our practice.
• Facilitate weekly clinical meeting for coordination of care with practitioners and health coach..
• Additional responsibilities may include participating in community events, providing educational resources to patients, and staying up-to-date with the latest research in functional medicine and hormone optimization.

Time Availability:

• This is a part-time position
• The practitioner will have some flexibility in their schedule, but must be available for patient appointments and team meetings as required.
• Must be available for communication through email and internal communication tools Monday - Friday, hours flexible
• This position is predominantly virtual and also may require occasional in-person work.

Pay: $55,000.00 - $88,400.00 per year
