Position Summary
The Mental Health Registered Nurse provides comprehensive psychiatric nursing care across two settings: a residential treatment facility and an outpatient behavioral health clinic. This dual-site role requires flexibility, strong clinical judgment, and the ability to adapt care delivery to meet patients at various points along the continuum of recovery. The nurse serves as a clinical liaison between the two programs, supporting continuity of care and coordinating smooth transitions for patients stepping down from residential to outpatient services.
Key Responsibilities
Residential Treatment (approximately 60% of schedule)
Conduct nursing assessments on admission, including mental status exams, physical health screening, and medication reconciliation. Administer and monitor psychiatric and medical medications, documenting responses and adverse effects. Collaborate daily with the multidisciplinary treatment team — including psychiatrists, therapists, and case managers — to develop and update individualized care plans. Monitor patient safety, respond to behavioral crises, and de-escalate using trauma-informed techniques. Provide psychoeducation to patients and families regarding diagnoses, medications, and coping strategies. Maintain accurate and timely documentation in the electronic health record (EHR).
Outpatient Services (approximately 40% of schedule)
Perform medication management appointments under the direction of a prescribing provider, including pre-visit vitals, symptom screening, and post-visit education. Conduct follow-up calls and check-ins with recently discharged residential patients to support transition and reduce relapse risk. Assist with care coordination, including referrals to community resources, primary care, and substance use programs. Participate in group programming support as clinically appropriate. Screen for crisis and facilitate emergency interventions or hospitalizations when needed.
Cross-Setting Responsibilities
Facilitate warm handoffs and transition planning between residential and outpatient levels of care. Participate in quality improvement initiatives, staff education, and program development. Maintain compliance with state nursing practice standards, HIPAA, and all applicable regulatory requirements. Attend required trainings, team meetings, and supervision as scheduled.

Work Environment & Physical Requirements
This position requires travel between residential and outpatient sites. The nurse must be able to sit and stand for extended periods, respond physically to behavioral emergencies, and work occasional evenings or weekends depending on the residential schedule rotation.
