Join an established care management practice serving Palm Beach County and northern Broward County as a Nurse Care Manager (RN or LPN). In this role you are the clinical lead for clients navigating medical, cognitive, or life complexity, wherever they call home: a private residence, an independent living or assisted living community, or a memory care setting. You assess, plan, advocate, and coordinate, bringing calm, clarity, and clinical precision to families during some of life's most difficult transitions. This is a care management role, not hands-on personal care, and not a hospital position. Your judgment and compassion directly shape each client's safety, dignity, and quality of life.
Duties
- Conduct comprehensive assessments (medical, psychosocial, and environmental) and build individualized, evidence-based Plans of Care
- Coordinate services seamlessly across health care, home care, and psychosocial needs
- Attend or facilitate medical appointments and advocate on behalf of clients
- Provide ongoing clinical oversight to keep services aligned with the Plan of Care
- Monitor for changes in condition and identify risks early, including fall prevention and medication management
- Assess home environments and recommend modifications that support safety and independence
- Maintain timely, accurate visit notes and documentation, and keep families informed with clear updates
- Collaborate with physicians, caregivers, agencies, and the client's legal and financial professionals
Requirements
- Current, active Florida Nursing License (RN or LPN) required
- Authorized to work in the United States
- 3 to 5 years in care management, case management, senior living, hospice, home health, or private duty
- Valid Florida driver's license, reliable transportation, and active auto insurance for travel to client homes and appointments
- Strong clinical judgment, discretion, and the ability to be a steady, trusted presence for anxious families
- Comfort working independently across multiple clients and priorities
- Proficiency with documentation and care management software (for example, Microsoft 365, Bill4Time)
- Bilingual English and Spanish a plus
