Nurse Case Manager FT/PT Flexible hours

We conduct Comprehensive Health Screenings, identifying Heart and Lung Disease, and early identification of Cancer. We are seeking a skilled and motivated RN Case Manager to join our healthcare team. As an RN Case Manager, you will play a crucial role in providing oversight to each department we serve in multiple states, ensuring no firefighter is left behind. Responsibilities

• Coordinating healthcare services, appointments, and managing a roster of patients that must be seen annually.
• Ensuring accurate and up-to-date documentation of patient information and care plans.
• Monitoring and evaluating patient progress and outcomes when a patient is deemed not fit for duty.
• Ensure timely and effective communication with patients, healthcare providers, and payers ( Fire or Law enforcement agencies)
• Report out weekly updates to the leadership team.
• Client communication and coordination of annual physical projects
• Bachelor's or Master's degree in Nursing.
• Certification in Case Management (CCM or ACM) preferred
• Experience in a similar role in healthcare settings.
• Familiarity with electronic health records (EHR) systems.
• Strong knowledge of healthcare regulations and compliance.
• Excellent organizational and time-management skills.
• Advanced clinical assessment and critical-thinking abilities.
• Ability to work independently and as part of a team.
• Strong communication and interpersonal skills.
• Strong working knowledge of Microsoft products, including Excel. You will use Excel and online software programs extensively.

• Starting Pay range is $45.00-$55.00 hour, based on experience
