Physical Requirements
The physical requirements for this position include regularly sitting, standing, walking, with some pushing, pulling, and/or lifting objects weighing up to 10lbs and occasional pushing, pulling, and/or lifting objects up to 50lbs.
Shift
Monday – Friday, Day Shift; primarily 8:00 am – 5:00 pm (40 hrs/wk); other hours/schedule as needed
Job Summary
The nurse clinical coordinator plays a vital role in coordinating patient care, acting as a liaison between patients, their families, and various healthcare providers.
Previous experience in cardiology and the care of patients with heart disease is preferred with specific knowledge of both cardiac and pulmonary disease.
This position works with Gill Heart and Vascular Institute Pulmonary Hypertension program, faculty, and team. Understanding therapies for pulmonary hypertension and right heart failure is essential.
Care coordination includes planning and support for tests, procedures, clinic visits, and ongoing care management per faculty orders. The Coordinator serves as contact for patient education, medication management, symptom exacerbation, and home therapies.
