GENERAL STATEMENT OF DUTIES

The Nurse Coordinator is a patient care provider. The Nurse Coordinator will be responsible for the management of physician orders to outside services to ensure accurate and timely care delivery. The Nurse Coordinator will also be responsible for managing the individualized patient service plans in accordance with physician orders and instructions as well as administrative procedures. This will be accomplished by coordinating transitions of care from referring facilities during admission and assist with a safe transition home. In addition, the Nurse Coordinator will fulfill the duties of a RN as needed. As a part of the Nurse Management team, this position will monitor compliance with regulatory agencies in the nursing and direct care programs and ensure these teams have the resources, skills, and tools needed to follow best practices and provide safe and effective care to the patients at Moody.

ESSENTIAL FUNCTIONS

Major Duties and Responsibilities:

  • Manage individualized service plans.
  • Process admissions and discharges by coordinating with referral sources, business development and admitting physician.
  • Ensure physician orders are acted on and communicated appropriately with pharmacy, lab, internal case managers and referral sources, including obtaining prior authorizations and diagnosis codes for tests.
  • Function as floor nurse as needed.
  • Perform cart audits to ensure EMR and medications on cart are accurate with current orders.
  • Prepare patient medications for patients going out on pass or discharging.
  • Schedule and perform patient medication education.
  • Responsible for bimonthly medication destruction with a second nurse.
  • Attend interdisciplinary rounds with physicians and team.
  • Assist with supply inventory and orders (i.e. Medline).
  • Coordinates with the Physician, Director of Nursing, and Program Directors to ensure Moody can meet the medical needs of the patients.
  • Ensure documentation integrity in the electronic health record (EHR) and adherence to HIPAA and facility policies.
  • Engage in root cause analysis investigations related to improving patient safety and outcomes, specifically in nursing care areas of responsibility.
  • Serve as a resource for patient and family education in conjunction with nursing, clinical, and direct care staff. This includes both informal and formal education as well as attendance at patient care meetings.
  • Assist with program development; helping identify appropriate equipment and ensuring appropriate staff education to be able to meet the needs of the populations served.
  • Support and assist in research efforts both of Moody employees and affiliates.
  • Other duties as assigned.

Minimum Qualifications:

Required Knowledge, Skills, and Abilities

  • Deep understanding of neurological disorders, post-acute rehab, and interdisciplinary care models.
  • Proven leadership, team building, and conflict resolution skills.
  • Excellent organizational, communication, and critical thinking skills.
  • Requires flexibility for occasional evenings, weekends, and on-call responsibilities.
  • Frequent interaction with patients, family members, and clinical staff in a hospital setting.

EDUCATION:

Current RN license in Texas

Associate’s degree as an RN required

EXPERIENCE:

Minimum of 1 year of inpatient experience required; rehab, neurology, and/or ALF experience preferred.

RN training and staff education experience preferred.

Bilingual in Spanish beneficial.
