The Adams County Health Department Nursing Division is comprised of Nurse Family Partnership, Nurse Support Program, Harm Reduction, Sexual Health, Immunizations, Child Maltreatment and Fatality Prevention, and Care Coordination for Children and Youth with Special Health Care Needs. The Nursing Division is made up of clinical support staff, medical assistants, harm reduction specialists, operations support, nurses, advanced practice nurses, and midwives.

Nurse-Family Partnership
Nurse-Family Partnership (NFP) is a program within Adams County Health Department Nursing Division that is a national, evidence-based maternal, infant, and early childhood home visitation program which provides fosters long-term success for first time pregnant and parenting people. Primary goals of the programincludeimprovedpregnancyoutcomes,optimalchildhealthanddevelopment,positive parenting, and family self-sufficiency.
The nurse home visitor in the NFP program:


  • Provides comprehensive, professional public health nursing services for low-income, high-risk women, birthing people, and their families, from early pregnancy to the child's second birthday.
  • Actively recruits and maintains a caseload of 25 families.
  • Maintains fidelity to the NFP program model.
  • Consistently meets program goals and objectives.
  • Supports client-centered goals.
  • Conducts thorough health assessments and targeted public health nursing interventions.

The Adams County Health Departments Nursing Division has Public Health Nurse classifications that have been graded into two tiers to provide employees with a career progression within that job title without further specialization into another job area. The Public Health Nurse I and II were created to provide employees with an opportunity for professional growth within the area to respectively represent entry level, journey level, and senior level accreditation within that job title as follows:

Nurse-Family Partnership Public Health Nurse I (PHN I)
The NFP Public Health Nurse I (PHN I) I is an entry-level position performing professional nursing process activities such as assessment, planning and evaluating public health care and treatment of birthing people and infants up to two years, within the scope of services provided at the Adams County Health Department. Must have completed an accredited Associate or Bachelor’s degree of Nursing program and has 1- 2 years of nursing practice. The PHN I understands program-level processes and interdepartmental roles, develops therapeutic relationships, and provides evidence-based education, support and resources for the community and clients seeking services. Participates in continuous quality insurance activities, and practice improvement activities such as Practice Council. The PHN I recognizes the Public Health tenants and the Public Health Nurse role and functions responsibly as required by the Public Health Nursing Scope and Standards of Practice.

Public Health Nurse II (PHN II)
The NFP Public Health Nurse II (PHN II) is a mid-level position that demonstrates advanced nursing process skills through nursing experience or competency in addition to those of the PHN I. Must have completed an accredited Associate or Bachelor’s degree of Nursing program and more than 3 years of nursing practice. In addition to the responsibilities noted above, this position requires the PHN II to have additional professional skills and experience in:
  • outcomes measurement,
  • delegation,
  • competency assessment,
  • teaching, precepting and mentoring,
  • training and on-boarding new PHNs
  • creative problem solving,
  • active engagement in continuous quality improvement,
  • public health and program model of care champion,
  • practice improvement leadership such as Practice Council Chair/Co-Chair,
  • accreditation liaison and
  • active participant on agency and community committees.

This position may be filled at the I or II level, depending on the selected candidate’s qualifications and experience.

NFP Nurse Home Visitor PHN I : $69,444.54 - $100,694.58
NFP Nurse Home Visitor PHN II : $78,027.88 - $113,140.43

Examples of Duties for Success

  • Develops and maintains therapeutic relationships with a diverse and complex client population, utilizing concepts of re?ection and motivational interviewing; modeling client-centered, strengths-based, and solution-focused approaches.
  • Recruits and enrolls a caseload of 25 active clients by 9-12 months post-hire and maintains Colorado caseload targets thereafter, per Colorado Caseload Maintenance Guidelines.
  • Continues to monitor and support client engagement in the program and proactively implements strategies to retain clients in the program.
  • Ensures and adheres to professional boundaries.
  • Formulates and implements plans for nursing and general health care for individuals and families based upon evaluation of needs and a focus on meeting mutually accepted health goals and objectives.
  • Delivers nursing services through home visits, case management and community outreach programs.
  • Completes comprehensive and holistic nursing assessments, including model speci?c assessments such as Dyadic Assessment of Naturalistic Caregiver-Child Experiences (DANCE), Intimate Partner Violence clinical pathway ASQs, PHQ-9, and GAD-7.
  • Formulates nursing diagnosis based upon nursing assessments, STAR Framework and client goals, and develops a plan of care with the client.
  • Assist clients in establishing realistic and achievable goals and measurable outcomes for promoting their health, caregiving abilities and life course development.
  • Plans home visits in accordance with client goals and NFP outcomes. Provides education and in support in assisting women and their families in attaining their targeted goals and educational needs.
  • Assists clients in ?nding resources to meet needs and makes referrals for services as appropriate.
  • Consults and collaborates with other professionals involved in providing services to women and families; provides timely and coordinated case management services with a variety of health, behavioral health, and social service providers.
  • Documents health history, assessments, goals and objectives, and care provided in compliance with department and program policy, procedures, and protocols.
  • Completes NFP clinical documentation thoroughly and in a timely manner.
  • Completes NFP data collection forms and associated assessments, per NFP required timelines and with a high degree of accuracy.
  • As required, completes Medicaid billing documentation per agency and state expectations and ensures that all billable services are submitted within timely ?ling guidelines.
  • Participates in review and analysis of the ETP reports to identify achievements and areas for improvement. Participates in quality improvement e?orts.
  • Maintains con?dentiality of information.
  • Attends and participates in NFP required weekly re?ective/clinical supervision sessions with NFP supervisor to facilitate individualized care planning for clients to facilitate optimal client outcomes and mitigate nurse home visitor secondary vicarious trauma.
  • Schedules, prepares for, and completes three joint home visits with NFP Nurse Supervisor each year with a regular cadence. Utilizes principles of re?ection to debrief on visit experiences.
  • Assumes responsibility for ongoing learning in relation to program implementation and actively engages in professional development to meet all NFP Nurse home Visitor competency requirements.
  • Attends and participates in team based re?ective practice by contributing to team meeting and case conferences discussions at both eh small team and large team levels.

  • Participates in evaluating ACHD nursing programs and services through documentation, routine statistical reporting, and team conferences.
  • Participates in individual, team, and programmatic review of NFP data and development and implementation of quality improvement strategies.
  • As requested/required, participates in NFP program evaluation e?orts at the agency, community, state and national levels.
  • Represents ACHD as a health resource in the community. Interprets public health programs and policies to individuals, families, community groups, schools, physicians, and other collateral community agencies and service providers.
Additional Duties:
  • Assists in the orientation and training of new employees, students, and interns.

  • Maintains electronic health record (EHR) proficiency and skills as necessary for daily duties.

Education & Experience:

NFP Nurse Home Visitor PHN I :

Requires a minimum of an Bachelor's Degree in Nursing (BSN) from a school accredited by the National League for Nursing Accrediting Commission or the American Association of Colleges of Nursing AND A minimum of one-year nursing experience, preferably in the area of adult, pediatric and community-based services. Clinical rotation hours, internships, and/or volunteer hours worked during the course of a BSN program count as qualifying experience. New graduates are encouraged to apply.

OR

Requires a minimum of an Associate's Degree in Nursing (ADN) from a school accredited by the National League for Nursing Accrediting Commission or the American Association of Colleges of Nursing AND A minimum of 3 years of nursing experience, preferably in the area of adult, pediatric and community-based services. Clinical rotation hours, internships, and/or volunteer hours worked during the course of a BSN program count as qualifying experience.

NFP Nurse Home Visitor PHN II:

Requires a minimum of an Bachelor's Degree in Nursing (BSN) from a school accredited by the National League for Nursing Accrediting Commission or the American Association of Colleges of Nursing AND a minimum of 3 years professional nursing experience, preferably in the area of Maternal Child Health and community-based services.

OR

Requires a minimum of an Associate's Degree in Nursing (ADN) from a school accredited by the National League for Nursing Accrediting Commission or the American Association of Colleges of Nursing AND a minimum of 5 years professional nursing experience, preferably in the area of Maternal Child Health and community-based services.

Professional Qualifications:


  • Licensed as a Registered Nurse (RN) in the State of Colorado or compact state. Must maintain active licensure throughout employment.
  • Current BLS certification
  • Demonstrated experience in Motivational Interviewing preferred.
  • Must pass background check

Other Qualifications:
  • Flexibility in work hours may be required to accommodate community events, meetings, and outreach activities.
  • Travel within the community and participation in evening, weekend and on call duties may be required.
  • This position requires a valid driver’s license and reliable transportation.
