Nurse Navigator for Medicare Chronic Care Management Program (LPN/RN)

Innova Primary Care is a Primary Care Clinic with locations in Huntsville and Madison, AL. We offer full service advanced primary care including urgent acute care same day visits, Annual Wellness Visits, Gyn care and in office procedures. We prioritize customer service for our patients to receive an excellent healthcare experience. We are open Mon-Fri 7:30AM - 5:30 PM and Saturday 9 AM - 12 PM.

Seeking an experienced LPN or RN to serve our patient guests in the role of Nurse Navigator.

Hours: Full Time, 40 hours/week

Job Description:

• You will be assigned a specific population of Medicare patients for whom you will serve as a Patient Advocate and Care Coordinator through the Medicare Chronic Care Management (CCM) program
• Coordinate care across different offices and facilities for patients.
• Handle day to day needs such as prescription refills, nurse triage, scheduling appointments.
• Problem solve for social work needs to ensure overall wellness of patient
• Be available to talk on the phone or by portal with your patients regarding their needs or health concerns
• Regular check-ins with your Medicare patients to ensure they are up to date with preventive services and compliant with medications, etc...
• Review documents from Specialist offices and update patient charts

Required Experience: Nursing experience minimum of 5 years. Preferred experience in rehab, long-term care, home health care or skilled nursing facilities.

Skills:

• Must enjoy talking on the phone and spending time with geriatric patient population.
• Computer/EHR skills (eClinicalWorks)

• Preferred knowledge or experience with the Medicare CCM program (https://www.cms.gov/medicare/payment/fee-schedules/physician-fee-schedule/chronic-care-management-complex-conditions) and comfortable documenting time as needed for billing purposes.Hourly pay rate based on license and years of experience.

Pay: $21.00 - $32.00 per hour
