Position Overview:
We are seeking a compassionate and reliable Part-Time Nurse (RN) to support our home care team. This role focuses on conducting Start of Care (SOC) visits, ongoing supervisory visits, and ensuring our clients—especially veterans—receive high-quality, compliant care.

This is an ideal opportunity for a nurse who values flexibility, autonomy, and meaningful patient relationships.

Key Responsibilities:

• Complete Start of Care (SOC) visits within required timeframes (typically within 72 hours of authorization)
• Conduct supervisory and follow-up visits in accordance with care plans and program requirements
• Develop and update individualized care plans
• Ensure compliance with VA and company documentation standards
• Communicate regularly with the care team regarding client needs, concerns, or changes in condition
• Provide education and guidance to caregivers to ensure quality care delivery
• Identify and report safety concerns, changes in condition, or additional service needs

Qualifications:

• Active RN license in the state of New Mexico
• Minimum 1 year of nursing experience (home care experience preferred)
• Strong clinical assessment and documentation skills
• Excellent communication and organizational abilities
• Ability to work independently and manage a flexible schedule
• Reliable transportation and valid driver’s license
