Job Overview:
We are looking for a compassionate and experienced Nurse Practitioner to provide primary and ongoing care to residents in a skilled nursing facility in Newark, NJ. The NP will work closely with physicians, nurses, and the interdisciplinary team to ensure residents receive high-quality medical care.

Responsibilities:

• Conduct routine and acute patient assessments for nursing home residents

• Manage chronic conditions and develop treatment plans

• Respond to changes in residents’ conditions and provide timely care

• Order and interpret diagnostic tests and labs

• Prescribe medications in accordance with New Jersey regulations

• Coordinate care with physicians, nursing staff, and families

• Ensure proper documentation in the EMR

• Participate in care plan meetings and regulatory compliance

Requirements:

• Active New Jersey Nurse Practitioner license

• Board Certified Nurse Practitioner (FNP, AGNP, or GNP preferred)

• Experience in skilled nursing facility (SNF), long-term care, or geriatrics preferred

• Strong clinical decision-making and communication skills

• Ability to work collaboratively with nursing home staff
