We are a dynamic multispecialty medical group located in Pleasanton, CA, committed to delivering high-quality, comprehensive care to our adult and senior patient population. As we continue to grow, we are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our outpatient team with a focus on Primary Care and Endocrinology or Sleep & Allergy.

Position Summary

The ideal candidate will provide full-spectrum primary and preventative care with a focus on chronic disease management, particularly for patients with diabetes and endocrine disorders. And the other opening is for Allergy & Sleep. These both include medication management, ongoing monitoring, and coordination of care with our internal care team and external specialists.

You will build expertise in managing complex endocrine patients while delivering general primary care in a supportive, team-based environment.

Key Responsibilities

• Perform comprehensive physical examinations and diagnostic evaluations

• Prescribe medications and implement care plans

• Monitor and manage chronic conditions, including diabetes, hypertension, and hyperlipidemia

• Collaborate with physicians and specialists on care coordination

• Educate patients on preventative care and wellness

• Accurately document care using our EMR system (Epic)

Qualifications

• Education:

  • Master of Science Degree from an accredited Nurse Practitioner or Physician Assistant program

  • Current California licensure (NP or PA-C)

• Preferred Experience:

  • Experience in outpatient primary care

  • Background working with adult and geriatric patients

  • Familiarity with Epic EMR system

• Skills:

  • Excellent communication and interpersonal abilities

  • Strong organizational and clinical decision-making skills

  • Ability to work independently and collaboratively within a team
