Nurse Practitioner

MedStreamline is seeking a Hybrid Nurse Practitioner (NP) with a background in Family Medicine, Internal Medicine, and/or Geriatric Care to join our expanding virtual care team. The ideal candidate is a self-motivated clinician who thrives in a tech-enabled environment and holds active licensure in multiple U.S. states. The NP will serve as a primary point of contact for patients across various states, reviewing patient data from our remote monitoring platforms, conducting telehealth visits, and supporting chronic condition management programs. Board certification in FNP, AGNP, or internal medicine is preferred.

The ideal candidate will hold active licensure in California and be willing to obtain additional state licenses as needed. Additionally, NPs must be eligible and willing to obtain the 103 NP certification issued by the California Board of Registered Nursing to practice independently without Physician supervision.

Candidates must reside in and be authorized to work in the United States. This is due to current licensure and operational requirements. Candidates must maintain a permanent residence in California for the duration of their employment.

Primary Responsibilities:

  • Perform comprehensive health assessments, including physical exams, medical histories, and medication reconciliation.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary care team, tailored to each resident’s needs and preferences.
  • Manage chronic and acute health conditions with timely interventions and appropriate referrals.
  • Order, review, and interpret diagnostic tests (e.g., labs, imaging) to support clinical decision-making.
  • Prescribe medications and treatments within licensure and in accordance with state laws and facility protocols.
  • Lead care initiation for residents enrolling in chronic care management services.
  • Educate and counsel residents and families on health maintenance, disease prevention, and self-care practices.
  • Work closely with physicians, nurses, therapists, social workers, and other healthcare staff to ensure integrated, high-quality care.
  • Participate in quality improvement efforts and interdisciplinary team meetings to optimize resident outcomes.
  • Document all patient assessments, treatments, and communications accurately and timely (within 72 hours) in the electronic medical record.
  • Support other duties as assigned in alignment with our products and services.

Candidate Profile:

  • Current, unrestricted Nurse Practitioner license in California; additional state licensure is a bonus.
  • Active 103 NP Certification, or eligibility and willingness to obtain it, to practice independently without Physician supervision.
  • Minimum of 3 years or 4600 hours of clinical experience in Family Medicine, Geriatric Care, and/or Internal Medicine
  • Board certification in at least one of the following subspecialities required: FNP, AGNP, or Internal Medicine preferred.
  • Strong clinical assessment, communication, and interpersonal skills.
  • Ability to work both independently and collaboratively in a team-based environment.
  • Competency in medication management and de-escalation techniques.
  • Proficient with electronic medical records and healthcare technology.
  • Willingness to travel occasionally within your assigned region to complete initial patient visits.

Travel Requirements: This role currently requires approximately 50% travel. Travel requirements may increase or decrease based on business needs.

Schedule: Monday–Friday, standard business hours. Occasional flexibility required for urgent patient escalations.
