Nurse Practitioner Job Description

We are seeking a motivated and compassionate Nurse Practitioner (NP) to join our clinical team. The NP will provide comprehensive family medicine services to patients of all ages, with a strong focus on preventive care, chronic disease management, hormone replacement therapy (HRT) (including men and women), and minor office procedures. This is an excellent opportunity to work in an innovative DPC model that allows for more time with patients, autonomy, and meaningful clinical relationships.

Key Responsibilities:

  • Provide comprehensive primary care across the lifespan, including pediatric, and adult populations.
  • Diagnose and treat acute and chronic illnesses with an integrative approach.
  • Manage and optimize hormone levels in both men and women using evidence-based protocols (e.g., BHRT, testosterone replacement).
  • Perform office-based procedures such as joint injections, skin lesion removal, Pap smears, biopsies, and others as trained.
  • Order and interpret appropriate lab work, imaging, and diagnostic tests.
  • Collaborate closely with the care team to ensure continuity and quality of care.
  • Educate and empower patients on preventive strategies, lifestyle interventions, and treatment options.
  • Maintain accurate and timely documentation in the EMR.
  • Participate in quality improvement initiatives and help shape protocols for a growing practice.

Qualifications:

  • Licensed Nurse Practitioner (NP-C or FNP-BC) in the state of Minnesota.
  • Certification in Family Practice is required.
  • 1–2 years of clinical experience in family medicine and urgent care
  • Experience or training in hormone replacement therapy (HRT) is highly preferred; willing to train the right candidate.
  • Proficient in performing minor office procedures or willing to be trained.
  • Excellent interpersonal skills and a passion for patient-centered care.
  • Comfortable working independently within a collaborative team.
  • Familiarity with direct primary care (DPC) or interest in this model is a plus.

Pay: From $120,000.00 per year
