We are a unique and rare provider in the US for very sick patients. We possess all clinical and technological services under one roof which can assist with disease management of the patient. Through Tele-Health, in-home care, Care Management (CMS) and Remote Patient Monitoring, we are providing quality of life for those that without us would be otherwise underserved. We take care of our patients at their home to ensure that we are collecting the most accurate information on medical and psychosocial conditions.

We are looking for a Family/Geriatric/Internal Medicine Registered Nurse Practitioner to join our senior management team in providing primary care, psychosocial social and care management to our patients. The ideal candidate will be well organized, flexible, and have a background prescribing and diagnosing with at least 2 years of experience.

We’re looking for a versatile and committed professional who can work independently as well as with our established medical team to ensure that our patients receive the care they need. We provide a vehicle for you to use if you are on the field. Our services are making a difference with those who are very sick. These specific types of patients are prone to not answer their phones and are less likely to see their doctor when they should because of access to care issues. There is a significant impact that CCMS’s Care Management program will have on “Best Care” practices for patients and savings for our partner providers. Controlling disease management and reduction of hospital stays, to have less impact on patients and reduce costs. Our struggle, as it is for all, is to effectively manage that 5% of patients, that is 40% to 60% of all the overall global costs.

Responsibilities

* Conduct telemedicine visits for Transitional Care Management (TCM) and Chronic Care Management (CCM) follow-ups.

* Provide comprehensive primary care for patients, incorporating the latest research in prevention.

* Assist hospitalists with patients being discharged, patient satisfaction from hospital and nursing homes.

* Assist with overall management of diseases and create Care Plans.

* Provide consultative services for patients and caretakers when more complex care is required.

* Manage a panel of regular patients.

* Examine, diagnose, and treat patients with a specialty in internal medicine.

* Collaborative Care Model (CoCM): This is an evidence-based approach that integrates behavioral health into primary care. It involves care coordination and regular consultation with psychiatric providers.

Duties

  • Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to inform diagnosis and treatment plans.
  • Manage acute and chronic conditions utilizing evidence-based practices while coordinating with physicians and specialists as needed.
  • Administer medications, injections, IV infusions, dermal fillers, botulinum toxin treatments, and perform procedures such as suturing and spinal taps with precision and safety.
  • Utilize EMR (Electronic Medical Record) systems like Epic, Cerner, or eClinicalWorks to document patient encounters accurately and maintain comprehensive medical records.
  • Provide patient education on disease management, medication adherence, nutrition, and lifestyle modifications to promote health literacy.
  • Assist with diagnostic evaluations including laboratory testing, sonography, medical imaging interpretation, and specimen collection/processing.
  • Support telehealth consultations and participate in case management activities to ensure continuity of care across outpatient and inpatient settings.
  • Engage in specialized areas such as hospice & palliative medicine, infectious disease care, occupational health, or senior care based on experience.
  • Ensure compliance with HIPAA regulations and maintain strict confidentiality of patient information at all times.

Schedule: Monday-Friday & Tuesday-Saturday

Work location: At the office (Miramar 33027)/on the field

Salary: 90.000-100.000/year. Bonus of 10.000 based on 80% productivity.

Standard shift: Day

Supplemental schedule: On-call, one week per month

Ability to commute in company-owned vehicle:

Dade/Broward County (We pay for gas allowance, gasoline)

License/Certification:

FNP/APRN and PMH-BC License (Required)
