Remote Patient Monitoring (RPM) Nurse Practitioner – Job Description

Position Summary:
The Nurse Practitioner (NP) provides high-quality, patient-centered care through Remote Patient Monitoring (RPM) programs. This role involves managing patients with chronic conditions by reviewing transmitted health data, conducting virtual visits, adjusting care plans, and collaborating with interdisciplinary teams to improve patient outcomes.

Key Responsibilities:

  • Monitor and interpret patient-generated health data (e.g., blood pressure, glucose, weight, pulse oximetry) through RPM platforms
  • Conduct telehealth visits to assess patient conditions and provide ongoing care
  • Develop, implement, and adjust individualized care plans for chronic disease management
  • Identify abnormal readings and intervene promptly to prevent complications
  • Educate patients and caregivers on disease management, medication adherence, and device use
  • Collaborate with physicians, nurses, and care coordinators to ensure continuity of care
  • Document patient interactions and clinical decisions in the electronic health record (EHR)
  • Ensure compliance with regulatory guidelines, including CMS RPM requirements
  • Support quality improvement initiatives and patient engagement strategies

Qualifications:

  • Active Nurse Practitioner license (state-specific; compact license preferred if applicable)

Pay: $40.00 - $50.00 per hour

Expected hours: 10.0 – 25.0 per week
