Position Details
Location: Ashland City, TN
Schedule: Monday–Friday, part-time (½ days)
Role Overview
As a Nurse Practitioner with Ultra Health Providers, you will serve as a trusted clinician for residents in your assigned facility. You’ll manage acute and chronic conditions, provide preventive care, and partner closely with nursing staff and administrators to ensure exceptional patient outcomes.
*** THIS POSITION SERVES TWO LOCAL FACILTIES: ONE IN AUBURN, KY AND ONE IN PEMBROKE, KY***
Responsibilities
- Deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settings
- Conduct admission assessments, physical exams, medication reviews, and depression screenings
- Collaborate with attending physicians, nursing staff, and facility leaders to coordinate care
- Counsel patients and family members on treatment options and changes in health status
- Manage clinical documentation accurately and in compliance with facility and state standards
Qualifications
- Active and unrestricted Nurse Practitioner license in Tennessee
- Current national board certification (AANP or ANCC)
- Minimum 1–2 years of experience in primary care, geriatric, or long-term care settings preferred
- Strong clinical judgment and communication skills
- Commitment to compassionate, patient-centered care
