We are currently seeking experienced and compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our growing team.

Key Responsibilities

• Conduct patient visits in homes, nursing facilities, assisted living communities, and hospice settings
• Assess, evaluate, and manage wounds with individualized treatment plans
• Perform physical examinations and review patient medical histories
• Order, interpret, and review diagnostic tests (e.g., labs, imaging)
• Diagnose and manage acute and chronic conditions
• Prescribe and manage medications in accordance with clinical guidelines
• Maintain accurate, timely documentation and manage patient schedules
• Educate patients and families on disease prevention and health management
• Collaborate with multidisciplinary teams to ensure optimal patient outcomes

Requirements

• Dependable, professional, and detail-oriented
• Flexible and adaptable to changing patient needs and schedules
• Compassionate bedside manner with the ability to remain calm under pressure

Qualifications

• Active and unrestricted California Nurse Practitioner or Physician Assistant license
• Current DEA registration required
• Clinical experience in hospital, clinic, ER, or similar healthcare setting
• Reliable transportation and valid auto insurance
• Strong communication and interpersonal skills
• Proficiency with electronic health records (EHR systems preferred)
• Multilingual candidates encouraged (Spanish, Armenian, Russian, or Chinese preferred)
