<Responsibilities>
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine diagnoses and treatment plans.
- Administer medications and injections, including IV infusions and botulinum toxin treatments, following established protocols and safety standards.
- Perform clinical procedures such as suturing, catheterization, spinal taps, and dermal fillers with precision and aseptic technique.
- Manage acute and chronic conditions across specialties like emergency medicine, geriatrics, pediatrics, behavioral health, and hospice & palliative medicine.
- Utilize electronic health record (EHR) systems such as Epic, Cerner, or eClinicalWorks to document patient encounters accurately and efficiently.
- Provide patient education on health coaching, medication management, nutrition, and disease prevention tailored to individual needs.
- Coordinate care through case management strategies for patients in assisted living facilities, nursing homes, or home care settings; ensure proper discharge planning and utilization review.
<Requirements>
- Valid Nurse Practitioner license with current state certification; additional certifications in family practice or specialized areas preferred.
- Proven experience in medical office settings with familiarity in EMR/EHR systems like Epic or Athenahealth.
- Strong clinical skills including physical examinations, diagnostic evaluation, ICD coding (ICD-10), CPT coding, and medical documentation accuracy.
- Experience with diverse patient populations such as pediatrics (including toddler care), geriatrics (memory care), or individuals with disabilities.
- Proficiency in performing procedures like venipuncture, sterile processing techniques, airway management, and specimen collection/processing.
- Critical care experience from ICU or Level I/II trauma center environments is highly desirable; hospital medicine or emergency room experience is a plus.
- Knowledge of infection control practices, HIPAA compliance, pharmacology including psychopharmacology and pain management strategies.
- Ability to work effectively in outpatient or inpatient settings with excellent patient assessment skills; familiarity with telehealth platforms is advantageous.
