Job Title: Mobile Nurse Practitioner – Therapy Services
Job Summary:
We are seeking a compassionate and adaptable Nurse Practitioner (NP) to provide therapeutic care through a mobile clinic van. This role focuses on delivering patient-centered therapy services—including rehabilitative support, wellness coaching, and basic physical therapy interventions—directly to patients in their homes or community settings. The ideal candidate thrives in a non-traditional environment and is committed to expanding access to care.
Key Responsibilities:
- Conduct patient assessments, including physical and functional evaluations
- Develop and implement individualized therapy and care plans within NP scope of practice
- Provide therapeutic services such as guided exercise, mobility support, and health counseling
- Travel to patient locations using a fully equipped mobile clinic van
- Educate patients on treatment plans, at-home exercises, and preventive care strategies
- Monitor patient progress and modify care plans as needed
- Maintain accurate documentation using electronic medical records (EMR)
- Ensure proper setup, use, and sanitation of all mobile medical equipment
- Collaborate with physicians, physical therapists, and other healthcare providers when necessary
- Adhere to all regulatory, safety, and organizational standards
Qualifications:
- Active and unrestricted Nurse Practitioner license
- Board certification (e.g., Family Nurse Practitioner or Adult-Gerontology NP)
- Experience or training in therapy, rehabilitation, or related field preferred
- Valid driver’s license with a clean driving record
- Strong clinical judgment and patient care skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage a mobile schedule
Preferred Skills:
- Background in home health, outpatient therapy, or community health
- Familiarity with therapeutic exercises and mobility training techniques
- Strong organizational and time-management skills
- Comfort working in a mobile and flexible care environment
Work Environment:
- Mobile clinic van traveling to patient homes and community sites
- Combination of driving and direct patient care
- Flexible schedule based on patient needs
Physical Requirements:
- Ability to lift and transport equipment
- Ability to assist patients with movement and exercises
