About Fractus Health

Fractus Health is building a telehealth platform starting with daily low-dose sublingual ketamine for depression and chronic pain, with compounded ketamine trochesprescribed via telehealth and shipped directly to patients.

We are looking for experienced Nurse Practitioners to support clinical care delivery as part of a growing telehealth practice. We are looking for strong clinical generalists who are comfortable working across multiple patient presentations, workflows, and care verticals in a structured telehealth environment.

Role Overview

As a Fractus Health provider, you will provide remote patient care within defined clinical protocols, collaborate closely with our Medical Director and broader clinical team, and help deliver safe, high-quality, efficient telehealth care at scale.

This role is ideal for NPs who combine:

• strong adult outpatient fundamentals,
• sound clinical judgment,
• comfort with telehealth and protocolized care,
• and the flexibility to work across evolving service lines.

What You’ll Do

• Conduct telehealth evaluations and follow-up visits for adult patients.
• Assess patient appropriateness for treatment using company protocols and clinical judgment.
• Document visits thoroughly and accurately in the EHR.
• Prescribe and manage treatment plans in accordance with state law, company policy, and supervising physician / Medical Director guidance.
• Escalate higher-risk, atypical, or complex cases appropriately.
• Collaborate with the Medical Director on safety, quality, and workflow execution.
• Support chart review, follow-up decision-making, and patient messaging as needed.
• Maintain strong compliance with telehealth and patient safety standards.
• Adapt across multiple clinical workflows / verticals as the platform expands

What We’re Looking For

• Active U.S. Nurse Practitioner license in good standing
• Board certification as an NP
• Ability to practice remotely for adult patients in a telehealth setting
• Strong generalist clinical foundation; not limited to a single niche specialty
• Experience in one or more of the following is required: primary care, family medicine, adult medicine, urgent care, pain, behavioral health, telemedicine, or other high-volume outpatient care
• Comfortable working within structured protocols while knowing when to escalate
• Strong documentation habits and attention to detail
• Professional, responsive, patient-centered communication style
• Comfort operating in a startup or evolving care model: we value flexibility / adaptability, understanding of fast paced start-up environments, and a willingness to change and grow in role and responsibilities as we evolve

Preferred Qualifications

• Multi-state licensure (minimum 5 states)
• Prior telehealth experience
• Experience with controlled-substance-adjacent workflows, compliance, and risk screening
• Active DEA number
• Experience treating mental health and chronic pain populations
• Familiarity with ketamine treatment is a plus, but not required
• Experience in async + sync care models is a plus
