Position Overview:
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to provide comprehensive primary care.

What You’ll Do:
1. Perform comprehensive medical evaluations and physical examinations in patients’ homes
2. Diagnose and manage acute and chronic medical conditions.
3. Develop and implement individualized care plans.
4. Prescribe medications and manage ongoing treatment.
5. Perform medication reconciliation and monitor for effectiveness and side effects.
6. Provide patient and caregiver education on disease management and preventive care.
7. Coordinate care with physicians, specialists, home health agencies, and other providers.
8. Order and interpret diagnostic tests, labs, and imaging as appropriate.
9. Maintain accurate and timely documentation in the EMR.
10. Identify changes in patient condition and intervene appropriately.
What We’re Looking For:
1. Active Texas Nurse Practitioner license.
2. Board certification (FNP, AGNP, or equivalent).
3. Active DEA license with prescriptive authority.
4. 1+ year of clinical experience preferred (primary care, geriatrics, or home health a plus).
5. Strong clinical assessment and decision-making skills.
6. Excellent communication and interpersonal skills.
7. Ability to work independently in a mobile care environment 8. Comfortable providing care in patient home settings.
