Nurse Practitioner

Serve geriatric patients in providing high-quality care in a clinic setting and home visits. Must be Bilingual in Chinese or Vietnamese

Responsibilities:

• Deliver patient care in the chicago area.
• Conduct annual health assessments in our new wellness clinic and some home visits.
• Travel between locations (costs covered).

Patient Load:

• 7 Patients in Clinic
• 3-4 Home visit

Requirements:

• NP license for California.
• Experience with geriatric patients and EMR systems (EPIC preferred).

Pay: $100,000.00 - $200,000.00 per year
