Nurse Practitioner

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team of dedicated healthcare professionals. As a hybrid NP, you will have the opportunity to work on-site in skilled nursing facilities while also working from home, supported by a dedicated scribe team for efficient documentation. Our comprehensive preventative care program incorporates Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to prioritize proactive measures for enhanced physical health and well-being.

Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home

• Utilize remote patient monitoring and chronic care management to detect abnormalities and provide timely interventions

• Collaborate with facility staff and our clinical team to ensure seamless patient care

• Maintain accurate and timely documentation with support from a dedicated scribe team

• Build strong relationships with facility staff and patients, maintaining a patient-centered approach

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills

• Ability to work independently and as part of a team
