Position Summary:
The Family Nurse Practitioner provides medical care to patients, including diagnosis and treatment of acute and chronic disease(s) as directed by a supervisory physician while placing an emphasis on preventative health and patient education.
Essential Functions:

Performs systematic physical examinations

Obtains medical and social history of patient

Accurately interprets lab data and other supporting information for accurate diagnosing and treatment

Appropriately documents patient data, diagnosis, and treatment plan
-
Appropriately codes patient visits
-
Requests appropriate follow-up visits from patients
-
Appropriately prescribes medications
-
Consults with supervising physician
-
Initiates medical and social referrals as indicated
-
Provides direction to ancillary staff
-
Actively attempts to solve problems and works towards common goals of the Center
-
Attends staff meetings, participates in continuing education

Assist supervising physician in providing optimal care for clinic patients

Other duties as required

Education, Experience and Licensure:

Current Indiana license and valid CSR/DEA required.

Nurse Practitioner certification preferred

Minimum or 5 years nursing experience with 3 years of supervisory/managerial experience in a healthcare setting
Physical Demands:

May sit and/or stand for long periods of time

May require full range of body motion including bending, stretching and kneeling or squatting

May occasionally be required to lift up to 50 lbs.

Must be able to see and hear in normal range with or without correction device(s)

Dexterity and hand to eye coordination as normally associated with operating office equipment, computers and telephones
