What You'll Own:

  • Serve as the primary medical provider on a Constella pod, delivering whole-person care to adults living with serious mental illness, in our clinic center, in the community, and via telehealth when appropriate
  • Conduct comprehensive health assessments that address both the physical and behavioral health needs our members bring to the door, recognizing that unmanaged chronic conditions, substance use, and untreated psychiatric illness are usually tangled together
  • Develop and manage longitudinal treatment plans for a panel of members, including management of common chronic conditions (diabetes, hypertension, COPD, hepatitis C, HIV) alongside stabilization of behavioral health needs
  • Provide medication management across physical and behavioral health, with close collaboration and oversight from our treating psychiatrist on complex psychotropic regimens
  • Work shoulder-to-shoulder with Community Health Workers, Social Workers, and Medical Assistants as part of an integrated pod, leaning on the team to meet members where they are
  • Coordinate closely with primary care practices, hospital systems, inpatient psychiatric facilities, and community providers to ensure members don't fall through the cracks during transitions of care
  • Contribute to the clinical model itself. You'll be one of our earliest clinicians in Kentucky, and what works (and doesn't) in your panel will shape how we train and support every NP who comes after you.
  • Document clinical work thoroughly and on time in our EHR, and partner with the team on quality and outcome measures
  • Quarterbacking the clinical performance of the pod. Ensuring everyone knows their role in driving clinical, behavioral, and social outcomes - and achieves them
  • Practice under a collaborative practice agreement with physician oversight; DEA registration and buprenorphine waiver (or willingness to obtain) required given our SUD population

Who We Need:

  • A nurse practitioner with strong primary care instincts who is drawn to, not intimidated by, the complexity of serving people with serious mental illness
  • Someone comfortable managing the physical health of a complex panel while partnering closely with a treating psychiatrist on the behavioral health side
  • A clinician who has seen acuity before, whether in primary care, emergency medicine, urgent care, street medicine, or a safety-net setting, and knows how to triage calmly
  • Someone comfortable working outside the four walls of a clinic, willing to see members in shelters, in their homes, or wherever trust can be built
  • A team-based clinician who sees CHWs, Social Workers, and MAs as essential partners, not support staff
  • Someone who can hold clinical rigor and human flexibility at the same time. Protocol-driven where it matters, improvisational where it has to be.
  • A clinician who wants to help build something, not plug into something already finished
  • Someone who thinks innovatively, and how technology can help them work smarter, faster, and drive better outcomes - a person who wants to contribute to the product design through operational leadership

What Success Looks Like:

  • A stable, engaged panel of members receiving consistent whole-person care across their physical and behavioral health needs
  • Better-managed chronic conditions, improved medication adherence, and measurable stabilization across the panel
  • Reduced avoidable ED and inpatient utilization for your members over time
  • Strong working relationships with your pod, with our treating psychiatrist, and with community partners who refer into Constella
  • Clinical feedback that materially improves how Constella delivers care over time

Your Superpowers:

  • 5+ years of clinical experience in primary care, urgent care, or a comparable setting serving complex, high-acuity adult populations
  • Experience caring for patients with serious mental illness, substance use disorders, or significant social complexity, whether through CMHC, CCBHC, FQHC, safety-net, street medicine, or Medicaid-focused practice
  • Comfortable managing common psychotropic medications with psychiatric backup, and eager to build that muscle further
  • Strong EHR and documentation habits
  • A collaborative, non-hierarchical disposition. You give and receive feedback easily.

Pay: $150,000.00 per year
