We are seeking a Nurse Practitioner or Physician’s Assistant to join a busy and growing family practice in Sussex County, New Jersey. We are looking for a practitioner who enjoys fostering long-term relationships with patients and their families and can embrace a team approach to patient care. Our practice is owned and operated by a physician who is double board certified in internal medicine and pediatrics and we provide comprehensive patient-centered care to our patients. At our practice, we not only take a whole-person, but a whole-family approach to healthcare, offering our patients individualized care and treating them with respect and dignity.

Essential responsibilities
- Functions independently to perform age-appropriate history and physicals and screening evaluations for well and sick patients.
- Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.
- Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities
- Implements interventions to support the patient to achieve health and wellness across the lifespan; including lifestyle, nutrition and exercise counseling for healthy patients and those with specific diseases and diagnoses
- Monitors the effectiveness of interventions.
- Facilitates the patient’s transition within and between health care settings, e.g. specialist referrals, hospital admitting, transferring, and general coordination of care.
- Collaborates with multidisciplinary team members
- Facilitates staff, patient and family decision making by providing educational tools
- Potential to either lead or co-lead our upcoming Diabetes Education program and/or Nutrition program
Minimum qualifications
- Graduate of an accredited Nurse Practitioner Program
- Current Advanced Practice Registered Nurse (APN) Licensure in NJ
- Current provider card in Basic Life Support (BLS)
- Strong computer skills, including Electronic Medical Record charting (EPIC), plus familiarity with Word, Excel
