Parrish Palliative and Hospice Care of Ohio is a nurse-owned, Joint Commission-accredited organization providing advanced illness care across Northwest Ohio. Our palliative care service supports patients living with serious, chronic, or progressive illness — focused on symptom relief, quality of life, and supportive care alongside curative or disease-modifying treatment.
Position Summary
We are seeking a compassionate and clinically skilled Nurse Practitioner (NP) to provide in-person palliative care consultations and follow-up visits to patients with serious illness. This role is centered on complex symptom management, advance care planning, and supportive care — delivered wherever the patient lives: home, assisted living, skilled nursing, or independent living communities. Patients on this service are not in hospice and continue to receive their regular medical care from their primary and specialty providers.
Key Responsibilities
- Conduct in-person palliative care consultations and follow-up visits in patient homes, ALFs, SNFs, ILFs, and other community settings throughout Northwest Ohio
- Perform comprehensive history and physical assessments, including symptom burden, functional status, psychosocial needs, and caregiver support
- Develop and manage individualized treatment plans for pain, dyspnea, nausea, fatigue, anxiety, insomnia, and other distressing symptoms
- Lead advance care planning (ACP) conversations, including POLST/MOLST completion, healthcare proxy discussions, and documentation of patient values and goals
- Provide chronic care management (CCM) and principal care management (PCM) services where appropriate
- Bill appropriately for E/M, ACP (CPT 99497/99498), and care management codes in accordance with CMS guidelines
- Coordinate closely with primary care physicians, specialists, facility staff, and family caregivers to align care plans
- Document clinical findings accurately and timely in the EHR
- Provide education to patients, families, and facility staff on disease trajectory, symptom management, and supportive care options
- Identify patients who may benefit from a transition to hospice and coordinate appropriate referrals with patient/family consent
- Adhere to all federal, state, and accreditation standards, including CMS, Ohio Board of Nursing, and Joint Commission requirements
Qualifications
Required:
- Master’s or Doctoral degree from an accredited Nurse Practitioner program
- Active, unrestricted Ohio APRN license with Certificate to Prescribe (CTP)
- National certification as FNP, AGNP, or AGACNP
- Active DEA registration (or willingness to obtain)
- Current BLS certification
- Valid Ohio driver’s license, reliable transportation, and current auto insurance
- Strong clinical assessment and complex symptom management skills
- Excellent communication skills, particularly in serious illness and goals-of-care conversations
Preferred:
- 2+ years of experience in palliative care, geriatrics, oncology, cardiology, pulmonology, or a related field caring for medically complex adults
- ACHPN certification (Advanced Certified Hospice and Palliative Nurse)
- Familiarity with palliative care billing (E/M, ACP, CCM/PCM, TCM)
- Experience with EHR documentation and home-based or community-based care delivery
- Experience caring for patients with Alzheimer’s disease and related dementias, advanced heart failure, COPD, cancer, or other serious illness

Pay: $55.00 - $65.00 per hour
