POSITION: Nurse Practitioner – Dementia and Elder Care Specialty
REPORTS TO: Medical Director, Palm Beach Elder Care
COLLABORATES WITH: Clinical Services Director, Care Coordination Team, Outreach Staff, Primary Care Physicians, Community Specialists

SUMMARY OF JOB RESPONSIBILITIES

The Nurse Practitioner (NP) at Palm Beach Elder Care is an advanced practice nurse who provides person-centered, dementia-competent primary care to elderly adults. The NP functions within the full scope of advanced practice nursing under Florida state law, working collaboratively with interdisciplinary teams to create, execute, and maintain patient-centered care plans.

The NP manages a panel of patients with dementia and other chronic illnesses, emphasizing wellness, symptom management, education, and prevention of avoidable emergency care. This role ensures high-quality, coordinated care while meeting clinical, operational, and regulatory performance standards.

ESSENTIAL RESPONSIBILITIES & TASKS

1. General Duties

  • Practices within the legal scope of NP practice in Florida.
  • Participates in on-call, weekend, and holiday rotations as needed.
  • Responds promptly to urgent care concerns and clinical inquiries.
  • Participates in quality assurance and improvement initiatives.
  • Maintains accurate, complete, and timely medical documentation.
  • Attends interdisciplinary case reviews and clinical meetings.
  • Provides clinical support and education to staff, caregivers, and community providers.
  • Cross-covers for palliative and home-based care as needed.
  • Completes other duties as assigned.

2. Clinical Care

  • Conducts initial and follow-up health assessments.
  • Develops and updates individualized care plans based on medical findings and patient goals.
  • Identifies and manages symptoms and conditions related to dementia and comorbidities.
  • Coordinates with PCPs and specialists for diagnostic and therapeutic recommendations.
  • Provides caregiver and patient education related to health, wellness, and disease progression.
  • Communicates effectively around prognosis, goals of care, and end-of-life decisions.
  • Upholds strict confidentiality in accordance with HIPAA and internal policy.

QUALIFICATIONS

  • Licensed Advanced Practice Registered Nurse (APRN) in Florida.
  • Board certified (preferably in Family or Geriatric Practice).
  • DEA license and prescriptive authority in Florida.
  • Meets at least one of the following dementia care qualifications:
  • ≥25% of panel aged 65+ or with cognitive impairment in past 5 years
  • Specialty training or board designation in geriatrics, neurology, psychiatry
  • Valid Florida driver’s license, reliable transportation, and current auto insurance.
  • Eligible for Medicare/Medicaid billing.

COMPETENCIES & REQUIREMENTS

  • Experience with older adult populations and dementia care.
  • Strong critical thinking and compassionate communication skills.
  • Ability to work independently in home, clinic, and virtual settings.
  • Competent in electronic medical records (EMR) and Microsoft Office.
  • CPR certified and maintains continuing education requirements.

PHYSICAL REQUIREMENTS

  • Must be able to lift/push/pull up to 50 lbs.
  • Requires frequent standing, walking, driving, and interacting in diverse environments.
  • Visual and auditory ability sufficient for assessments and communication.

WORK CONDITIONS

  • Work is performed in clinic settings, patient homes, and occasionally in external facilities.
  • Potential exposure to environmental hazards and infectious materials.
  • Must maintain safety standards in varied home settings.

Job Types: Full-time, Part-time, Contract

Pay: $30.00 - $35.00 per hour

  • Geriatrics: 3 years (Required)
  • Dementia care: 3 years (Required)
