Nurse Practitioner (Fluent in Spanish)

*** Must be License Nurse Practitioner in New York AND/OR Conneticut ***

The NP plays a vital role in providing remote clinical care for patients with multiple chronic health conditions. The clinician is involved in completing telehealth visits with new and active patients for a medical provider that offers case management and remote monitoring services. Essential duties and responsibilities include:

• Complete an E/M visit for new patients and determine the appropriateness for the chronic care management and remote patient monitoring services
• Obtain medical history, conduct virtual exam, and complete risk assessments based on the medical and behavioral health conditions
• Recommend treatment plan for follow-up by nursing staff and case management team
• Provide education by communicating with the patient, family member and/or caregiver
• Engage with other health care providers on the patient’s behalf with the goal of reducing emergency room visits and/ or hospitalizations
• Address escalated patient vitals and measurements
• Provide clinical support by offering consultation, education information around conditions and interventions in order to maintain focus on outcomes and best practices
• Submit evaluation and progress notes, prescriptions, and other related clinical documentation in an organized, timely and accurate manner as per policy and procedure
• Provide outreach and engagement of the patient to ensure clinical follow-up services are provided
• Provide crisis intervention when necessary
• Perform other related duties as assigned

REQUIRED QUALIFICATIONS

• Active NP state license in good standing in NY with independent prescriptive privileges
• Multiple state licenses strongly preferred
• Bilingual: Spanish/English

SKILLS AND/OR EXPERIENCE REQUIRED

• Experience working in virtual care/telemedicine
• Experience working with the geriatric population
• Experience in providing competent clinical services to patients with chronic health conditions or other disabilities
• PECOS certification preferred
• Effective interpersonal, communication and teamwork skills required
• Experience with harm reduction and motivational interviewing techniques
