Our structured, team-based approach allows providers to deliver deeper care and achieve measurable impact.We are seeking a bilingual Nurse Practitioner who wants to practice at the top of their license and serve our Hispanic community with culturally competent care.What You’ll Do Manage Type 2 diabetes and related metabolic conditions

• Optimize insulin, GLP-1, and oral therapies
• Monitor A1C and metabolic markers
• Educate patients in English and Spanish
• Collaborate with nutritionists and health coachesQualifications Active Texas NP license (FNP or AGNP)
• Fluent in English and Spanish (Required)
• Experience in primary care or diabetes management preferredWhy Join Us
