Vitalis Primary Care PLLC is seeking a compassionate and motivated Nurse Practitioner to join our growing adult primary care practice serving patients in Adult Family Homes and community-based residential settings.

Duties
The Nurse Practitioner will provide longitudinal primary care, chronic disease management, post-hospital follow-up, urgent visits as needed, medication management, and care coordination for adult and geriatric patients residing in Adult Family Homes and similar residential settings.
- Deliver comprehensive primary care to adult and geriatric patients in community-based residential settings
- Perform assessments, diagnosis, treatment planning, and medication management
- Manage chronic illnesses, acute changes in condition, and transitional care needs
- Coordinate with caregivers, facility staff, specialists, pharmacies, labs, home health, and ancillary services
- Document clearly and efficiently in the EMR
- Support a proactive care model aimed at reducing avoidable hospitalizations and improving continuity of care
- Participate in practice development and quality improvement as the organization grows
Qualifications
- Active Washington State Nurse Practitioner license in good standing
- National board certification as a Nurse Practitioner
- DEA registration and prescriptive authority, or ability to obtain
- Experience in adult primary care, geriatrics, home-based care, SNF/LTC, post-acute care, or complex chronic disease management preferred
- Strong clinical assessment and independent decision-making skills
- Excellent communication and care coordination skills
- Valid driver’s license and reliable transportation
- Willingness to travel locally within a 30-mile radius
