<Overview>
We are seeking a compassionate and highly skilled Nurse Practitioner specializing in hospice and palliative medicine to join our dedicated team. The successful candidate will play a vital role in providing comprehensive end-of-life care to patients in their homes, ensuring comfort, dignity, and support for both patients and their families. <Responsibilities>
- Conduct thorough patient assessments to determine eligibility for hospice services and develop individualized care plans.
- Manage symptom control and pain management strategies, including medication administration and IV infusion therapies.
- Collaborate with interdisciplinary teams to coordinate patient care, discharge planning, and family education.
- Provide clinical leadership in managing complex cases involving acute pain, behavioral health issues, or terminal conditions.
- Document all patient interactions accurately within electronic health record (EHR) systems such as Epic or eClinicalWorks, ensuring compliance with HIPAA regulations.
- Offer telehealth consultations when appropriate to extend care access and support continuity of treatment.
- Educate patients and families on disease progression, medication use—including Botulinum toxin treatment when applicable—and advanced directives.
<Requirements>
- Valid Nurse Practitioner license with certification in hospice and palliative medicine or related specialty.
- Proven experience in hospice or palliative care settings, with familiarity in managing end-of-life symptoms compassionately.
- Strong clinical skills including patient assessment, medication administration, and IV infusion techniques.
- Experience working with EMR/EHR systems such as Epic, Cerner, or Athenahealth; telehealth experience is preferred.
- Knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation processes relevant to hospice care.
- Ability to work independently while collaborating effectively within multidisciplinary teams across various healthcare environments including home care, nursing homes, hospitals, or assisted living facilities.
- Excellent communication skills with an emphasis on family-centered care; experience working with diverse populations including children and seniors is desirable.
