NURSE PRACTITIONER - HOSPICE OF SAN ANGELO

Primary functions are to assist in overseeing the clinical care and treatment of patients and families: and to consult with the patient's attending physician. in consult with the hospice physician: to provide clinical direction with the Interdisciplinary team to assure quality care. Organization and planning is necessary, along with the ability to explain plans and goals to others. Need for knowledge and understanding of the Agency policies, procedures, and philosophies.

  • Provide services in accordance with hospice.
  • Ensures informed consent, understanding of choices for care, and expected outcomes are explained and understood by patient/family if not already done.
  • Completes face-to-face encounters with patients, as assigned, prior to 3rd benefit period and each period to follow under the direction of a hospice physician.
  • Serves as attending physician upon request.
  • Adheres to policies regarding protected health information.
  • Consults with and educates patient/family in the disease process, self-care, prevention strategies, end of life care, including physical/psychological aspects.
  • Collaborates with IDT’s development of patient/family plan of care to assure quality care.
  • Provides clinical assessment results to the hospice physician after face-to-face encounters.
  • Performs and documents visits to patients/family in a timely.
  • Coordinates services with other professional and ancillary staff.
  • Informs the physician and other staff of the patient’s condition and needs.
  • Assists the director in IDT conferences.
  • Assists in the development of IDT plan of care.
  • Maintains close communication with patient’s physician, reporting all changes in status and collaborates with the physician to develop treatment protocols.
  • Assists in education on hospice-related items to staff as needed.
  • Assists with admissions, on-call, staff, and community education.
  • Provides a safe and comfortable environment.
  • Maintain the patient’s and family’s right to privacy and confidentiality at all times.
  • Works non-judgmentally with patients and families of various cultures, religious, and socioeconomic backgrounds and lifestyles.
  • Deals effectively with difficult situations by using negotiation skills to resolve conflict.
  • Attends weekly IDT meetings.
  • Exhibits good listening and comprehension.
  • Treats patients, families, and staff with respect and compassion.
  • No gossip, does not harm others in conversation.
  • Performs other responsibilities as assigned.

This position requires endurance to perform tasks over long periods of work hours and the ability to perform essential functions without posing a "direct threat" in the workplace. Must be able to lift 55 pounds.

Current license to practice as an RN in the State of Texas and as an Advanced Practice Nurse. Minimum of 2 years of medical-surgical or home health/hospice experience preferred.
