Hearts in Hands CPR & Services LLC is a Phoenix-based, community-focused healthcare organization expanding its network of Nurse Practitioners (NPs) to provide in-home and telehealth services for patients with chronic and complex medical conditions.
We are seeking experienced, compassionate, and self-motivated Nurse Practitioners who are already enrolled as individual AHCCCS rendering providers to join our growing clinical team.
This role focuses on delivering advanced, patient-centered care outside of traditional clinical settings, improving access to care, reducing hospital readmissions, and supporting vulnerable populations through timely evaluation, diagnosis, and treatment in the community.

Responsibilities
- Perform comprehensive in-home medical evaluations and follow-up visits
- Conduct telehealth consultations, assessments, and care management
- Diagnose, treat, and manage acute and chronic conditions within scope of practice
- Develop and implement treatment plans and care strategies
- Prescribe medications and therapies as appropriate
- Provide patient and caregiver education
- Document all encounters accurately and in compliance with AHCCCS standards
- Collaborate with physicians, case managers, and interdisciplinary teams
- Identify early clinical deterioration and intervene appropriately
- Conduct thorough patient assessments, including physical examinations, vital signs, and detailed medical histories to inform diagnosis and treatment plans.
- Manage acute and chronic conditions across various specialties such as emergency medicine, geriatrics, pediatrics, behavioral health, and primary care.
- Administer medications and injections, including IV infusions and botulinum toxin treatments, ensuring proper documentation in electronic health record (EHR) systems like Epic or eClinicalWorks.
- Perform procedures such as suturing, catheterization, spinal taps, and sterile processing techniques following aseptic protocols.
- Coordinate with multidisciplinary teams for discharge planning, case management, utilization review, and telehealth consultations to optimize patient outcomes.
- Educate patients and families on health coaching topics including medication management, nutrition, memory care, and post-acute care strategies.
- Ensure compliance with HIPAA regulations and maintain accurate medical records while adhering to NCQA standards for quality assurance.
Skills
- Active Nurse Practitioner (NP) license – State of Arizona
- Must be an active AHCCCS enrolled provider (Individual Rendering Provider)
- Active NPI (Type 1)
- DEA license (if prescribing)
- Current BLS certification (ACLS preferred)
- Minimum 1–2 years of clinical experience as an NP
- Valid driver’s license and reliable transportation
- Ability to travel locally for home visits
- Ability to practice independently within scope
- Proficiency in EMR/EHR systems including Epic, Cerner, Athenahealth, and eClinicalWorks for documentation and clinical workflows.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), CPT coding for procedures and billing purposes.
- Skilled in performing diagnostic evaluations using laboratory testing, sonography, medical imaging techniques, and specimen collection/processing.
- Expertise in managing complex cases involving infectious disease care, pain management (including acute pain), hospice & palliative medicine, occupational health, and behavioral health issues.
- Ability to perform triage assessments in urgent care or emergency room settings with a focus on airway management and ventilator support when necessary.
- Familiarity with specialty procedures such as dermal fillers, Botox treatments for esthetic laser therapy or therapeutic purposes.
- Demonstrated capability in working with individuals with developmental disabilities or dementia-related conditions like Alzheimer’s disease or memory care needs.
