Job Title: Nurse Practitioner (NP) – Home Health Care

Location: Atlantic County, NJ

Salary: $110,000 – $130,000 per year

Schedule: Full-Time | Flexible Schedule

Position Overview:

A growing home health care provider in Atlantic County is seeking a Nurse Practitioner (NP) to join their team and provide high-quality care to home-bound patients. This role is ideal for a provider who is passionate about improving patient outcomes and delivering personalized care in a home setting.

The Nurse Practitioner will work independently, managing a daily patient load while building strong relationships with patients and their families.

Key Responsibilities:

• See approximately 5–6 patients per day in a home setting
• Perform comprehensive assessments including vitals, diagnosis, and treatment planning
• Prescribe and manage medications in accordance with clinical guidelines
• Maintain accurate and timely documentation and scheduling
• Educate patients and families on disease management, prevention, and overall health practices
• Collaborate with care teams to ensure continuity and quality of care

Qualifications:

• Active New Jersey Nurse Practitioner license
• DEA certification
• Malpractice coverage
• Prior experience as an NP in clinic, nursing home, or ER settings
• Strong clinical judgment and ability to work independently
• Excellent communication and interpersonal skills
