Nurse Practitioner or Physician Assistant

Summary

We are seeking a dedicated Nurse Practitioner or PA to join our team in providing exceptional rehabilitative care at U.S. Physiatry. This role is vital in enhancing patient outcomes and ensuring high-quality service delivery within our network of facilities.

This role will be mainly functioning within the Skilled Nursing setting, with some occasional shifts in the Inpatient Rehab setting.

Responsibilities

  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Administer treatments and therapies as prescribed by physicians.
  • Monitor patient progress and adjust care plans accordingly.
  • Educate patients and families about treatment options and health management.
  • Maintain accurate medical records and documentation.
  • Participate in quality improvement initiatives to enhance service delivery.
  • Ensure compliance with healthcare regulations and standards.
  • Qualifications
  • Master's degree in Nursing or related field.
  • Current Nurse Practitioner or PA license in the state of practice.
  • Strong clinical skills with experience in acute or post-acute care settings.
  • Proficiency with electronic medical record systems (e.g., Epic).
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a team environment.
  • Experience with telehealth practices is a plus.
  • Knowledge of discharge planning processes.
