The Opportunity

This role offers the chance to work side-by-side with experienced PM&R physicians in a collaborative, interdisciplinary environment. It's a strong fit for a clinician who wants to focus on functional recovery, continuity of care, and measurable patient outcomes--without the burden of on-call responsibilities.

You'll play a key role in helping patients transition successfully through rehabilitation while working with a team that values coordination, consistency, and long-term impact.

What You'll Do

• Partner with a PM&R physician to manage patient care in skilled nursing and sub-acute rehab settings
• Collaborate closely with therapy teams (PT/OT/ST) to align medical care with functional recovery goals
• Coordinate with primary care providers, nursing staff, social workers, and families to ensure continuity of care
• Provide ongoing evaluation and management with a focus on pain control, mobility improvement, and reducing hospital readmissions
• Complete timely and accurate documentation, with flexibility to chart remotely

What We're Looking For

• Licensed Nurse Practitioner or Physician Assistant (required)
• Active certification as an NP or PA (required)
• Current Michigan license (required)
• Ability to work independently while collaborating across disciplines
• Strong communication skills and a patient-centered approach to care
• Reliable transportation to travel between facilities
