Nurse Practitioner/ Physician's Assistant

Job Summary
We are seeking a dynamic and compassionate Nurse Practitioner/Physician assistant to join our healthcare team, dedicated to delivering exceptional patient care across diverse clinical settings. In this role, you will serve as a vital link between patients and physicians, providing comprehensive assessments, diagnosing conditions, and developing personalized treatment plans. Duties

  • Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine health status and needs.
  • Diagnose acute and chronic illnesses across multiple specialties such as internal medicine, geriatrics, pediatrics, emergency medicine, and behavioral health.
  • Develop and implement individualized treatment plans, including medication management, injections, suturing, and wound care.
  • Perform procedures like IV insertions, catheterizations, dermal fillers.
  • Coordinate patient care through case management, discharge planning, utilization review, and collaborating with specialists in areas like gastroenterology, infectious disease care, hospice & palliative medicine, and occupational health.
  • Utilize electronic health record (EHR) systems such as Epic or eClinicalWorks for accurate documentation of patient encounters and medical records.
  • Provide telehealth consultations to expand access to quality care while maintaining HIPAA compliance and ensuring patient privacy.

Requirements

  • Valid Nurse Practitioner license with current certification to practice in the state of employment.
  • Demonstrated experience with EMR/EHR systems.
  • Strong clinical skills with experience with family practice,pediatric to geriactric.
  • Proficiency in performing procedures including suturing, venipuncture (phlebotomy), catheterization, tube feeding management, and sterile processing techniques.
  • Knowledge of CPT coding (Current Procedural Terminology), ICD-10 coding standards for accurate billing and documentation.
  • Experience working with diverse patient populations including pediatrics, geriatrics, individuals with disabilities or developmental disabilities, assisted living residents, nursing home patients, or those receiving home care services.
  • Strong understanding of infection control protocols and aseptic techniques necessary for safe practice across multiple clinical settings.
  • Excellent communication skills for effective patient education on topics such as nutrition, memory care (including Alzheimer’s/dementia), pain management strategies, and health coaching initiatives. Job Types: Full-time, Part-time

Pay: $44.00 - $54.00 per hour

Expected hours: 30.0 – 40.0 per week
