Schedule: Monday–Friday, 9:00 AM – 5:00 PM

Compensation: Starting at $32+ per note

About the Role

Seeking a compassionate and skilled Nurse Practitioner to manage chronic diseases in geriatric patients through our Remote Patient Monitoring (RPM) program. In this role, you will oversee patients’ ongoing care, ensuring their chronic conditions are closely monitored, managed, and optimized for better health outcomes.

Key Responsibilities

· Manage patients’ chronic diseases, including hypertension, diabetes, congestive heart failure, COPD, asthma, and obesity.

· Maintain a daily schedule of approximately 30 geriatric patients.

Patient Population

· Primarily geriatric patients managing multiple chronic conditions.

· Focused on improving health outcomes, quality of life, and disease management.

Requirements

· Active New York medical license (required)

· DEA registration with a New York address (required)

· Florida license (preferred)

· Prior RPM or chronic disease management experience strongly preferred

· Reliable internet connection — full remote setup and equipment provided
