Job Title: Nurse Practitioner

Rosenfeld Van Wirt, P.C. is an independent medical practice focused exclusively on patients in the long-term care setting. The practice was founded in 2013 to specialize in the care of post-acute patients in nursing homes, skilled nursing, rehabilitation and other long-term facilities. Rosenfeld Van Wirt, P.C. operates in over 50 facilities in Pennsylvania and New Jersey

Position Summary: This position is in Skilled Nursing Facilities (SNF). Our nurse practitioners work collaboratively with the facility’s care team to oversee both short- and long-term residents. Responsibilities include admissions, discharges, monthly care visits, and evaluations for changes in condition.

Requirements:

Previous nursing or clinical experience in long-term care

Strong communication and teamwork skills
