Job Overview
We are seeking a dedicated and energetic Nurse Practitioner to join our team in providing exceptional care within a Skilled Nursing Facility (SNF). This full-time position, scheduled Monday through Friday, offers an exciting opportunity to deliver comprehensive healthcare services to diverse patient populations. As a vital member of our healthcare team, you will utilize your clinical expertise to improve patient outcomes, support interdisciplinary collaboration, and ensure high-quality, compassionate care. Your role will involve performing assessments, managing acute and chronic conditions, and coordinating care plans tailored to each individual’s needs.
Responsibilities
- Conduct thorough patient evaluations, including physical examinations and vital signs assessments, to determine health status and identify medical issues.
- Develop and implement personalized care plans in collaboration with physicians, nursing staff, and other healthcare professionals.
- Manage acute pain, perform IV infusions, and administer injections while adhering to sterile techniques and safety protocols.
- Utilize electronic health record (EHR) systems such as Epic or eClinicalWorks to document patient encounters accurately and efficiently.
- Provide triage services by assessing urgent needs, prioritizing cases, and coordinating timely interventions for residents.
- Perform procedures including suturing, catheterization, spinal taps, and wound care with precision and adherence to infection control standards.
- Educate patients and their families on health management strategies, medication administration, nutrition, and disease prevention.
- Collaborate with interdisciplinary teams on discharge planning, case management, and utilization review to optimize patient outcomes.
Qualifications
- Current Nurse Practitioner license with valid certification in your state; experience working in a Skilled Nursing Facility preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic or Athenahealth; telehealth experience is a plus.
- Strong clinical skills including physical examinations, diagnostic evaluation, medication administration, and patient assessment.
- Experience with complex medical conditions such as geriatrics, dementia care, hospice & palliative medicine, infectious disease care, and acute care settings like ICU or trauma centers.
- Knowledge of CPT coding, ICD-10/ICD-9 coding standards for documentation purposes.
- Ability to perform procedures such as venipuncture, phlebotomy, sterile processing techniques, and basic life support (BLS).
- Excellent communication skills for effective collaboration with healthcare teams and patients’ families; familiarity with HIPAA regulations essential.

Pay: $100,334.39 - $125,166.14 per year
