Job Summary
Synergy Medical Group is seeking a dedicated, compassionate, and skilled Nurse Practitioner to join our growing team. In this role, you will provide high-quality, patient-centered care to individuals with acute and chronic health conditions, including those with developmental disabilities. You will work collaboratively within a multidisciplinary team to support patients in achieving optimal health outcomes and an improved quality of life. Key Responsibilities
- Perform comprehensive health assessments to identify the medical needs, goals, and challenges of individuals with developmental disabilities
- Develop, implement, and evaluate individualized, evidence-based treatment plans
- Provide direct clinical care, including support with activities of daily living as appropriate
- Administer medications and treatments in accordance with established protocols and provider orders
- Collect, process, and document specimens for laboratory testing
- Collaborate closely with physicians, nurses, therapists, and other healthcare professionals to coordinate care and monitor patient progress
- Participate in discharge planning and facilitate smooth transitions to community-based resources and services
- Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory and organizational standards
