Position Overview

We are seeking a bilingual Nurse Practitioner licensed in both Florida and New York to join our telehealth clinical team. The ideal candidate is compassionate, proactive, and comfortable practicing independently within a structured virtual care environment.

This is a remote full-time role with a competitive annual salary of $157,000 and opportunities for growth and leadership.

Responsibilities

• Provide virtual visits for Medicare patients across multiple care programs (CCM, BHI, PCM, TCM, RPM) or telehealth visits.
• Perform comprehensive assessments, follow-ups, and medication management
• Collaborate with supervising/collaborating physicians and the care management team
• Review patient data, monitor trends, and intervene when clinically necessary
• Document visits, care plans, and interventions in the EHR.
• Participate in monthly clinical meetings and quality improvement initiatives
• Support continuity of care following hospital or SNF discharges

Qualifications

• Active Nurse Practitioner (NP) licenses in both Florida AND New York (required)
• Bilingual: English + Spanish preferred (English + Ukrainian/Russian also considered)
• Experience in primary care, family medicine, geriatrics, or chronic disease management
• Previous telehealth experience is a plus but not required
• Strong communication, organizational, and documentation skills
• Comfortable working independently and managing a virtual patient panel

Schedule

• Full-time or Part time (minimum 15hours/week)
• 1099 position
• Monday–Friday
• Flexible schedule options

Compensation & Benefits

• $157,000 annual salary for 35 hours of work a week
