Duties

• Conduct thorough virtual patient assessments, including vital signs, medical history review, and symptom evaluation using secure telehealth platforms.
• Develop personalized treatment plans aligned with best practices and current clinical guidelines, including managing acute and chronic conditions.
• Utilize EMR (Electronic Medical Records) systems like Epic and Athenahealth to document patient encounters accurately and efficiently while ensuring HIPAA compliance.
• Provide health coaching, medication management, and follow-up care tailored to individual needs across various specialties including geriatrics and senior care.
• Collaborate with multidisciplinary teams to coordinate referrals for diagnostics like sonography or laboratory testing; assist with discharge planning and case management.
• Stay current with medical coding standards such as CPT (Current Procedural Terminology), ICD-10/ICD-9 coding systems for accurate billing and documentation.

Qualifications

• Valid Florida Nurse Practitioner license with current certification to practice independently in a telehealth setting.
• Proven experience in telehealth services with familiarity using EMR/EHR systems such as Epic, Cerner, or eClinicalWorks.
• Strong clinical background
• Demonstrated proficiency in performing cognitive assessments and developing supporting care plans for patients on medicare.
• Knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation techniques relevant to senior patients on medicare
• Experience working with individuals with disabilities and or chronic conditions.
• Excellent communication skills for effective triage and patient education; ability to work independently while collaborating within a multidisciplinary team.
