Job Summary:
We are seeking an experienced and compassionate Nurse Practitioner (NP) to join our growing team in a hybrid role providing both Primary Care and Urgent Care services. This position offers the opportunity to deliver comprehensive, patient-centered care across the continuum, managing chronic conditions while also evaluating and treating acute illnesses and injuries.

Responsibilities:

Provide comprehensive primary care services for pediatric (if applicable), adult, and geriatric patients.

Perform annual wellness visits, preventive screenings, and routine physical examinations.

Diagnose, treat, and manage acute and chronic medical conditions.

Develop individualized treatment plans and provide patient education.

Order, interpret, and communicate laboratory and diagnostic test results.

Prescribe medications and monitor treatment effectiveness.

Coordinate specialty referrals and follow-up care.

Evaluate and treat patients presenting with acute illnesses and minor injuries.

Diagnose and manage conditions including respiratory infections, influenza, COVID-19, urinary tract infections, skin conditions, gastrointestinal complaints, musculoskeletal injuries, and other urgent medical concerns.

Perform minor office procedures including wound care, laceration repair, incision and drainage, splinting, foreign body removal, and abscess management (within scope of practice).

Interpret basic diagnostic testing including EKGs, point-of-care testing, and imaging results as appropriate.

Determine appropriate disposition, including emergency department referrals when indicated.

Maintain accurate and timely documentation in the electronic medical record (eClinicalWorks preferred).

Collaborate with physicians, specialists, nursing staff, and care management teams to ensure coordinated patient care.

Participate in quality improvement initiatives and population health programs.

Ensure compliance with organizational policies, evidence-based clinical guidelines, and regulatory requirements.

Educate patients and families on disease prevention, wellness, medications, and treatment plans.
Qualifications:

Master's or Doctorate degree from an accredited Nurse Practitioner program.

Current New York State Nurse Practitioner license.

National Board Certification (AANP or ANCC).

Current DEA registration and New York State Controlled Substance Registration.

Minimum of 2 years of Nurse Practitioner experience in Primary Care, Family Medicine, Internal Medicine, Urgent Care, or Emergency Medicine.

Current BLS certification.

Strong clinical assessment, diagnostic, and procedural skills.

Excellent communication and patient education abilities.

Experience with eClinicalWorks (eCW).

Bilingual (Spanish or another language) preferred.

Experience treating patients across all age groups.
