As we expand our services into the Sioux Falls region, we are looking for a dedicated Nurse Practitioner who values independence, clinical excellence, and the opportunity to make a real impact within the community.

Position Overview

We are seeking a skilled and compassionate Nurse Practitioner to provide mobile wound care across Sioux Falls and surrounding areas. This PRN role offers flexibility, autonomy, and the opportunity to practice meaningful, one-on-one patient care in nontraditional settings. The provider will visit skilled nursing facilities, assisted living communities, memory care facilities, and private homes to deliver comprehensive wound assessments and treatments.

The Nurse Practitioner will work independently in the field while collaborating regularly with our Medical Director and participating in monthly clinical calls to ensure alignment with best practices and regulatory standards.

Key Responsibilities

Clinical Care

  • Perform comprehensive assessments for patients with acute and chronic wounds, including pressure injuries, diabetic ulcers, venous/arterial ulcers, surgical wounds, traumatic wounds, and infection-related wounds.
  • Develop, implement, and adjust individualized treatment plans using evidence-based wound care practices.
  • Perform wound care procedures, including cleansing, selective and non-selective debridement, dressing applications, compression therapy, and negative pressure wound therapy (NPWT/VAC) management.
  • Monitor wound progression and document healing outcomes.

Education & Communication

  • Educate patients, caregivers, and facility staff on wound care, prevention, and follow-up protocols.
  • Promote compliance with treatment plans through ongoing communication.

Collaboration

  • Work with the collaborating physician and clinical leadership during monthly team meetings and as needed to discuss patient care and treatment planning.
  • Communicate with home health agencies, hospice teams, and facility staff to ensure care continuity.

Documentation & Compliance

  • Document all encounters accurately and promptly within the EMR.
  • Use proper coding for wound care services in accordance with Medicare and organizational guidelines.
  • Maintain compliance with HIPAA, infection control standards, and state practice regulations.

Working Conditions

  • Mobile position requiring travel between patient locations within Sioux Falls and nearby areas.
  • Care delivered in patient homes and various long-term care settings.
  • Requires adaptability, clinical independence, and comfort practicing in nontraditional environments.
  • Ability to drive in varying weather conditions is essential.

Qualifications

  • Active and unrestricted Nurse Practitioner license in the practicing state; multi-state license a strong plus
  • National Board Certification (FNP, AGNP, or equivalent).
  • Reliable transportation, valid driver’s license, and active auto insurance meeting state requirements.
  • Minimum of 1 year of direct patient care experience as a Nurse Practitioner (home health, long-term care, or mobile practice preferred)
  • Demonstrated wound care experience, including assessment, staging, treatment, and ongoing management of acute and chronic wounds (pressure injuries, diabetic ulcers, venous/arterial ulcers, surgical wounds, etc.)
  • Strong clinical assessment and critical-thinking skills with the ability to identify wound etiology, complications, and appropriate treatment pathways
  • Ability to develop, implement, and modify individualized, evidence-based care plans in collaboration with interdisciplinary care teams
  • Knowledge of advanced wound care modalities, including debridement, negative pressure wound therapy, off-loading, compression therapy, and topical treatment selection
  • Familiarity with documentation standards and regulatory requirements, including Medicare guidelines, medical necessity, and compliance best practices
  • Experience working with care coordinators, home health agencies, long-term care facilities, and referring providers
  • Excellent documentation and EMR/EHR proficiency, with attention to detail and accuracy
  • Strong communication skills, with the ability to educate patients, caregivers, and facility staff on wound care plans and prevention strategies
  • Ability to work independently in the field while maintaining accountability, professionalism, and timely follow-up
  • Reliable transportation and willingness to travel within assigned service areas
  • Commitment to patient-centered care, quality outcomes, and continuous clinical improvement
  • Ability to meet physical requirements, including navigating stairs, working in varied environments, and lifting up to 50 pounds.

Compliance Expectations

  • Provider must practice within the South Dakota Nurse Practice Act and follow HHAWC clinical guidelines.
  • Strict adherence to HIPAA and confidentiality standards is required.
  • Compliance with OSHA, infection control, and any facility-specific requirements (such as TB testing or immunization documentation) is expected.
  • This PRN position does not guarantee a minimum number of hours; scheduling is based on patient needs.

Job Types: Part-time, PRN, Per diem

Pay: $50.00 - $60.00 per hour
