We are a mobile wound care practice delivering advanced, patient-centered wound care across Houston and surrounding communities. Our team provides specialized services directly to patients in homes, assisted living facilities, and in partnership with home health agencies.

Position Overview

We are seeking a skilled and independent Nurse Practitioner (NP) to join our team on a PRN basis.

This role is ideal for a provider experienced in procedural wound care, including debridement and biologic graft placement, who is comfortable working autonomously in a mobile setting.

Schedule & Workload

• PRN (as-needed)
• Typically 10–25 visits per week based on availability and patient volume
• Monday to Friday schedule
• Flexible daily scheduling within assigned visits

Key Responsibilities

• Perform comprehensive assessments and management of acute and chronic wounds
• Conduct procedures including sharp debridement, wound bed preparation, and advanced dressing applications
• Perform amniotic stem cell graft placement and biologic skin substitute applications
• Evaluate patient eligibility for advanced biologic therapies in accordance with Medicare guidelines
• Develop and implement individualized treatment plans
• Collaborate with physicians, home health agencies, and interdisciplinary teams
• Provide patient and caregiver education on wound care and prevention
• Maintain accurate, timely, Medicare-compliant documentation

Compensation & Benefits

• $75–$90 per visit (based on experience and complexity)
• Higher reimbursement for advanced procedures (grafts, debridement)
• Mileage reimbursement provided

Territory

• Houston, TX and surrounding areas including:
• Katy, Cypress, West Houston, Sugar Land, Pearland, Pasadena, and nearby regions
• Patients are typically grouped geographically for efficiency

Qualifications

• Active and unrestricted Texas Nurse Practitioner license
• Minimum 2 years wound care experience strongly preferred
• Experience with sharp debridement, NPWT, and grafting/biologics preferred
• Wound care certifications (WCC, CWCN, WOCN) a plus
• Strong clinical judgment and ability to work independently
• Reliable transportation and willingness to travel locally
