Responsibilities
- Conduct comprehensive patient evaluations and manage acute and chronic conditions.
- Order and interpret diagnostic studies, prescribe medications, and coordinate referrals as needed.
- Provide preventive care, health maintenance, and patient education.
- Collaborate closely with physicians, nurses, behavioral health consultants, and support staff to ensure continuity of care.

Maintain accurate, timely clinical documentation in accordance with organizational standards.
- Flexible scheduling options designed to support work–life balance, including built-in administrative time for full-time Advanced Practitioners to support charting and care coordination.
- On-site behavioral health consultant available multiple days per week, providing outpatient therapy and serving as a clinical resource for providers.
- On-site laboratory services and full-time phlebotomist, allowing patients to complete lab work conveniently before or during appointments.
- Recently renovated facility with private provider offices and dedicated exam rooms, supporting efficient workflows and a professional clinical environment.
